A successful transition home starts prior to your discharge from the hospital, rehabilitation facility, skilled nursing facility, or even your doctor’s office. Following a request from your physician, a registered nurse will meet with you and your family members, evaluate your needs, and establish a plan of care.

Things to consider when choosing a home healthcare or private duty agency:

  • Is the agency accredited by either CHAP or JCAHO?
  • Does the agency conduct thorough background checks on all employees prior to hire?
  • Does the agency provide continuing education and training for its clinical staff members?

HealthCall will assess your home to ensure it is readied for your recovery. HealthCall can also arrange for delivery of certain medical supplies and equipment as prescribed by your physician.
